How to read the matrix
The further top-right a risk sits, the more urgent it is. The score bands are a common starting point; adjust them to your organisation’s risk appetite if needed.

Five levels distinguish more finely without getting complicated. For small projects a coarser scale is fine too.
How often should I update the matrix?
At least at every milestone and whenever something significant changes.
